Homeless Recuperative Care Program

Our Homeless Recuperative Care program is designed to provide a clean, safe environment for individuals with nowhere to recover following hospital discharge.

Our Program's Mission:

Leverage the expertise of our Nursing and Program staff to greatly reduce communal homelessness through the provision of essential housing wraparound services.

Referral Process & Eligibility Criteria

Referring medical agencies may submit a referral for a homeless client via email or in person. The client will then be assessed by Martha’s Registered Nurse to determine the clients’ eligibility to access the program. Once eligibility is confirmed and it is determined that the client is able to complete all required activities independently, our Registered Nurse will coordinate the client’s admission to the Recuperative Care Program.

Members entering Recuperative Care must be:

  • 18 years of age or older
  • Homeless or residing in places not meant for human habitation, etc.
  • Able to complete all activities of daily living (ADLs) independently
  • Able to self-administer medication with 24-hour staff oversight
  • Intact mental status (i.e., alert and oriented to name, place, date, and situation)
